Community of Practice
Welcome to the Champlain Regional Stroke Network's Virtual Community of Practice!
In response to requests from our regional sites, we have created Virtual Communities of Practice (VCoPs) as forums for professional collaboration, information sharing, and connecting with peers to advance the provision of quality stroke care in the Champlain Region.

Evidence-based, key guidelines related to stroke care that are endorsed by Health Quality Ontario.
Stroke Prevention Clinics
The role of a Stroke Prevention Clinic (SPC) is to ensure that individuals who are at high risk for stroke receive evidence-based care founded on best practices. SPCs provide an integrated, comprehensive, interdisciplinary approach to stroke prevention. The main objectives are to reduce delays and inefficiencies in risk factor management of high-risk stroke patients and to facilitate timely access to interventions.
